What office policies should I be aware of?
Your appointment time is reserved just for you. A late cancellation or missed appointment leaves an opening in the therapists’ day that could have been filled by another patient. If you need to cancel or reschedule your appointment we require patients to inform Choice by 1pm, 1 business day prior to the appointment or there will be a $50 fee. Patients who miss an appointment will be charged the full appointment fee.
Our therapists have the right to terminate and/or refuse treatment if there is a reasonable cause (ie.: verbal, physical, or sexual harassment, including sexualization of treatment.). There is a zero tolerance policy for any kind of harassment at Choice Health Centre.
It is the responsibility of the patient to know and understand their insurance benefit coverage and limits. Clinic staff cannot contact insurance companies to get any information regarding your policy as it is private health information.
Payments for services rendered are the responsibility of the patient and due at the time of service. Any portion of the service fees not covered by insurance are the responsibility of the patient and due at the time of service.
